Thank you for considering the Medi-Share program. We are excited that you have decided to become part of our family!
Here is how to apply in four easy steps:
1) Enter your personal information.
2) Choose your program and enter vital information about you and your family which includes height and weight of each family member.
3) Submit your non-refundable application fee of $50 using your VISA/MasterCard/Discover credit or debit card*.
4) Receive confirmation of your application and follow up instructions.

*If you are approved for Medi-Share, there is also a one-time $120 membership fee and a one-time $5 ACCU membership fee payable with your first month's share. The $120 membership fee is waived for Senior Assist members.
Non U.S. citizens: Please note that if any applicant in your household is a non-U.S. citizen, your application will not be processed until a copy of the green card or visa is received by Application Services. You can fax a copy of this information to: (321) 722-5134.
Current Medi-Share members who need to "Add On" a family member do not need to complete this application. Please see the forms page from the member center for the correct application.
Medi-Share 1.0 members who would like to switch to Medi-Share 2.0 do not need to go through the application process---instead, follow the few simple steps to switch programs online at “Switch Now.”
