ACCOUNT SET UP
After you have clicked on “See a Doctor Now” from the CCM Member Center, you will need to complete the following steps to set up your account and start using the MDLIVE service.
NOTE: If you are not logged in as the Head of Household you will not be able to access the MDLIVE setup process.

Part 1: Activate Your Account
Step 1
You will land on an MDLive patient registration screen. Select a username, password and set security questions.

Step 2
The next screen will confirm that your account registration is complete. Click the green button labeled, “Access Your Account”.

Step 3
Sign in with the username and password you created. These are the same credentials you will need to access the MDLIVE app.

Part 2: Provide Health Information
Now that you’ve activated your account and signed in, you will need to provide basic medical information that can be made available to your MDLIVE doctors during consultations.
Click on the “My Health” tab from your dashboard to access personal health information. You must complete the “My Health History” section tabs before consulting with MDLIVE doctors.

Step 2
Add Your Primary Doctor (optional)

Step 3
Select a Preferred Pharmacy (optional)
NOTE: “My Health” is also where you can access history and records from MDLIVE consultations.
