Christian Care Ministry's mission is to connect people to a Christ-centered community wellness experience based on faith, prayer, and personal responsibility.

Registration form used for church groups and other organizational groups interested in Medi-Share. These groups must affirm that the organization is in agreement with the Christian Care Ministry Statement of Faith and hereby understand that enrollment in this program provides Christian Care Ministry the right to acknowledge the organization's affiliation.

Your CCM ID is listed next to the "CCM #” label on your member ID card. You can also locate your CCM ID in your Forms Center on the "Registration" tab below your contact information.
